Our facility is a single building on Lead Avenue SE in Albuquerque, New Mexico, that holds the entire continuum of care. Clients can move from medical detox to residential care and on to outpatient treatment without leaving the Ascend system, because every level of care lives under one roof.
The environment is built for comfort and safety. With 18 beds and around-the-clock nursing, the setting is intimate enough for individual attention and equipped for medically supervised care. This page explains how the building is organized and what to expect from the spaces inside it.
How is the building organized?
The building is organized so that higher levels of care sit upstairs and step-down care sits downstairs, all in the same location. This layout is what lets a client move between levels of care as a short walk rather than a transfer to a new facility.
| Location | What happens there |
|---|---|
| 883 Lead Ave SE, upstairs | Medical detox and residential treatment, with 24/7 LPN nursing on site |
| 881 Lead Ave SE, downstairs | Clinical, day treatment, and intensive outpatient care |
What nursing and medical care is on site?
Around-the-clock nursing is the core of our detox and residential care. During detox and residential treatment, licensed practical nurses are on site 24 hours a day. Detox begins with a full nursing assessment, a medical provider sees detox patients within hours of arrival, and withdrawal is monitored closely using the CIWA and COWS scales so care can be adjusted as the body stabilizes.
This level of medical oversight is what allows us to care safely for people withdrawing from alcohol, opioids, and other substances on site, rather than sending them elsewhere for stabilization. It is one of the reasons the Albuquerque location can offer the full continuum of care in one place.
What are the wellness spaces used for?
Our campus includes dedicated wellness spaces where clients take part in yoga, sound healing, breathwork, and mindfulness practices. Wellness spaces are areas that support the therapeutic work and help clients build healthy routines during treatment.
These are wellness activities, and they complement rather than replace the clinical hours of a client's treatment plan. The rooms are designed to feel calm and welcoming, giving clients a place to reset while nursing and clinical teams remain close at hand for the medical side of care.
What is the setting like during detox and residential care?
The setting during detox and residential care is calm, closely supervised, and built around comfort. With only 18 beds, the environment is small enough that staff know each client, and nursing coverage runs 24 hours a day. Detox patients remain on site for the duration of stabilization unless there is a medical emergency, which keeps care consistent and safe through the most vulnerable days.
Everyday routines are handled with dignity. Smoking and vaping are permitted outside only, during designated breaks. New detox and residential admissions have a five-day phone blackout, with phone access on day six with approved contacts, which helps clients focus on stabilization early in treatment. For residential clients, a van is available for medical appointments and court obligations with the proper releases, so treatment does not force people to fall behind on the rest of their lives.
